Most actions, such as claims submission, account updates, and investment changes, can easily be accomplished after logging in or with our mobile app, HRAgo®, without you having to submit a paper form. If you prefer, forms are available after logging in or upon request from our customer care center.

Click here to download a PDF copy of the Claim Form. Standard claims processing time is five to seven business days from the day we receive your claim. If you are not enrolled in direct deposit, remember to allow adequate time to receive your paper check reimbursements in the mail.
